چکیده

Objective: Osteoma is a benign, osteogenic tumor that usually grows slowly and asymptomatically. Giant ethmoidoorbital osteoma is a rare event that can give rise to ocular signs and symptoms; in such a case treatment is mandatory, although debates persist regarding the optimal approach. Methods: Between 1999 and 2007, six patients (four men and two women) with giant ethmoido-orbital osteomas were treated using endonasal endoscopic approach. Their ages ranged from 17 to 63 years (mean 46.3 yr). All procedures were performed under general anaesthesia using 0 ° and 30° nasal endoscopes. The osteoma was fragmented using intranasal drill and then the divided parts were detached from surrounding structures and pulled out of the nose. Finally the surgical site was packed. Results: The transnasal endoscopic approach was successful in all six patients. Follow up period ranged from 12 months-8 years (mean 46 months). There were no major complications and only one patient had postoperative persistent epiphora which was treated by endoscopic DCR and insertion of silicon tube. Conclusion: In selected cases of ethmoido-orbital osteomas, endonasal endoscopic approach provides a safe and effective alternative to open approaches, offering cosmetic advantages and lowering the morbidity.

Endoscopic Removal of a Giant Ethmoid Osteoma with Orbital Extension

Osteomas are slow growing bony tumours of the paranasal sinuses. They are usually asymptomatic but they may present with headache, cerebral symptoms, or visual disturbances, depending on their anotomical location. A computerized tomography scan is the imaging modality that should be choosen for the diagnosis of osteomas. Radiographically, osteoid osteoma appears as an opaque lesion with a nidus...

Orbital Extension of a Giant Ethmoidal Sinus Osteoma in a 30-Year-old Female

Introduction: Osteoma is a benign tumor rarely found in the paranasal sinuses.   Case Report: A 30-year-old female presented with an 8-month history of proptosis of the right eye that was progressing slowly. She was diagnosed with ethmoid osteoma and managed with collaborative surgery at the ophthalmology and otolaryngology departments. After surgery, the patient suffered visual loss that was m...

Giant osteoma of the ethmoid sinus with orbital extension: craniofacial approach and orbital reconstruction

Osteomas are the most common fibro-osseous lesions in the paranasal sinus. They are benign tumours characterized by slow growth and are often asymptomatic. Treatment is indicated in sphenoid osteomas that threaten the optic canal or orbital apex and in symptomatic cases. The choice of surgical management depends on the location, size and experience of the surgeon. An open approach allows tumour...

An insight into endoscopic transethmoidal resection of intraorbital tumors

This study aims to evaluate indications, complications and surgical technique in the endonasal endoscopic approach to orbital tumor resection. In this retrospective review, 11 patients who admitted and underwent endoscopic orbital surgery were reviewed. Demographics, preoperative and postoperative eye examination, and size and imaging characteristics of the tumours were tabulated. The endonasal...

Combined simultaneous transcranial and endoscopic endonasal resection of sphenoorbital meningioma extending into the sphenoid sinus, pterygopalatine fossa, and infratemporal fossa

BACKGROUND Sphenoorbital meningiomas are surgically challenging because of their nature to extend to adjacent structures. Here, we describe a case of recurrent sphenoorbital meningioma extending into the sphenoid sinus, pterygopalatine fossa, and infratemporal fossa, which was resected using combined simultaneous transcranial and endoscopic endonasal approaches. CASE DESCRIPTION A 62-year-old...
